The story begins when newly graduated Scottish doctor Nicholas Garrigan (James McAvoy) flies to Uganda. His visit to the exotic country promises him exciting experiences, romance and the joy of being able to help where people really need him. Shortly after his arrival, he is called to the scene of an accident - the new president, Idi Amin, has crashed his Maserati into a cow. Garrigan resolves the situation and wins Amin over with his unabashed directness. Amin, who was already obsessed with Scottish history and culture, takes an instant liking to Garrigan and offers to become his personal physician.
It's an offer the young doctor can't refuse - and it's here that his journey into the shocking behind-the-scenes of one of Africa's most horrific horrors begins. Garrigan begins to succumb to his own lust for power and becomes the dictator's confidant, advisor and right-hand man, witnessing increasingly disturbing events - kidnappings, murders and unspeakable monstrosities in which he himself may be complicit. When, in the throes of Amin's murderous megalomania, he finally tries to stop the madness that surrounds him, his stay turns into a desperate struggle for survival.
