Captain Benjamin Franklin Pierce, M.D., nicknamed Hawkeye (Donald Sutherland), is transferred with Captain Augustus Bedford Forrest, M.D., nicknamed Duke (Tom Skerritt), to MASH 4077, a mobile, Army surgical hospital. They have no military morale at all, don't acknowledge the military and like to joke around, drunkenness is not unknown among them and the nurses can be confident.
Neither their commanding officer, Colonel Henry Braymore Blake, M.D. (Roger Bowen), nor the unit's clerk, Corporal Walter Eugene O'Reilly, called Radar (Gary Burghoff), can handle them. They are joined by Pierce's friend Captain John Francis Xavier McIntyre, M.D., called Trapper (Elliot Gould). Then all three manage to get their arch enemy, Major Frank D. Marion Burns, known as the Kunny Face.
And they still annoy Major's big sister Margaret O'Hulihan (Hulihan), a favorite of Sorrel, Hrm (Sally Kellerman) and Lieutenant Dish (Jo Ann Pflug), as well as the unit's priest, John Francis Patrick Mulcahy (Rene Auberjonois). Before Hawkeye and Duke leave for home, they and Trapper manage to pull off a mock suicide of the camp dentist, try to get Korean boy Ho John (Kim Atwood) out of the Army, operate on a congressman's son while thinking about golf, play American football with the 325th Evacuation Hospital but also tough shifts in the operating room...
